Medicare: Our office accepts assignment on all Medicare claims. In order to accept assignment, we must have a current signature on file and a copy of your Medicare card. Medicare will reimburse our office 80% of the allowable charge. You or a secondary insurance company are responsible for the 20%.

PPO Plans: We participate in many PPO plans. Please inquire with our business office if you do not see our physician’s name listed as a provider with your insurance company. Patients are responsible to insure that their primary care physician has submitted the appropriate referral.

Other Insurance: The filing of primary or secondary insurance is performed as a courtesy for all patients. Please provide us with a copy of your current insurance card at the time of your visit.

St. John's Medical Group requires that a Patient Information form be completed at each visit. If there are any changes in your insurance coverage, please notify our office immediately. Regardless of insurance coverage, you are responsible for any fees incurred for services provided.
